Please follow the steps below to begin the Warranty Repair process.
1) Complete this form. Upon clicking the “Submit Warranty Repair” button, your information will populate the address fields at the FedEx website for label printing.
2) Print and retain a copy of your order form including ticket and tracking numbers (next page); include a copy of the order with your footwear inside the box.
3) Print the FedEx label and tape to a box (next page).
4) Drop your order at any FedEx box or pick-up location.
